29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Commercial rooftop, west side; On or about October 15, 2024, an employee was not protected from falls while engaged in roofing construction activities from a 5:12 pitch rooftop and near edges that ranged between approximately 10-feet to 18-feet, 5-inches above ground-level.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) Commercial rooftop, various locations; On or about October 15, 2024, employees were not effectively protected from falls while engaged in roofing construction activities from a 5:12 pitch rooftop and near edges up to 9-feet, 8-inches above ground-level, in that 50-foot vertical lifelines with rope-grab mechanisms were not properly adjusted to ensure free fall distances would be limited to no more than 6-feet.
