29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Building Phase 5: Administrative Office:  On or about October 16, 2024, and at times prior thereto, small appliances such as a Sunbeam microwave, model SGDJ701, were plugged into relocatable power taps and not used according to listing and labeling instructions. Employee(s) were exposed to potential electrical shock and/or fire hazard(s).

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  Maintenance and Mechanic Shops: On or about October 16, 2024, and at times prior thereto, the employer failed to conduct a workplace hazard assessment and create a written certificate for employees who conducted painting operations and equipment preventive maintenance operations which required safety glasses/goggles and chemical resistant gloves, which potentially exposed employees to contusions, lacerations, corneal scratches and burns.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  Phase 4: Maintenance Building: On or about October 16, 2024, and at times prior thereto, a Tailift forklift, model FD20, was modified with coverings over the falling object protection (FOP) structure that obscured the line of sight above the vehicle.  Employee(s) were potentially exposed to struck-by hazards.

29 CFR  1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  Phase 4: Maintenance: On or about October 16, 2024, and at times prior thereto, compressed air used for cleaning irrigation equipment was not reduced to less than 30 p.s.i. The air was approximately set to 50 p.s.i. This condition potentially exposed employee(s) to cutaneous or subcutaneous laceration(s)/embolism hazard(s).

29 CFR  1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift:  Breakrooms and Restrooms: On or about October 16, 2024, employees from Severson Group, a janitorial contract employer, poured and diluted eye hazardous chemicals into unlabeled secondary spray bottles and cleaned the employee break areas and restrooms.  The employer had not ensured proper warning labels were attached to the secondary containers which potentially exposed employees to eye irritations.
