29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Machine shop, SN 14351; On or about October 17, 2024 and times prior, employees were using a Swift lathe to repair equipment. The lathe was not equipped with a chuck guard, exposing employees to struck by hazards from flying chips. b) Machine shop, located near the bathroom; On or about October 17, 2024 and times prior, employees were using an American lathe to repair equipment. The lathe was not equipped with a chuck guard, exposing employees to struck by hazards from flying chips. c) Machine shop, located near the entrance to the shop; On or about October 17, 2024 and times prior, employees were using a Cincinnati drill press to drill holes in parts. The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips. d) Machine shop, located across from the bathroom; On or about October 17, 2024 and times prior, employees were using a Summit drill press to drill holes in parts. The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips.

29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces. a) Saw room; On or about October 17, 2024 employees used circuit breaker panels to turn on lights and equipment. The area around the panels were blocked with parts and pallets exposing employees to tripping hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Saw room; On or about October 17, 2024 employees use a breaker panel to turn lights on and off. The panel was not installed per manufacturer's instructions in that the panel door was missing exposing employees to electrical hazards. b) Saw room; On or about October 17, 2024 employees operating a horizontal band saw while standing or sitting in front of electrical panels. The panel on one breaker panel was not installed per manufacturer's instructions in that the panel door was open, exposing employees to electrical hazards.
