(b) On October 16, 2024, and at times prior, at the RapidXchange facility in Brookshire, Texas, the employer did not follow Recognized and Generally Acceptable Good Engineering Practices (RAGAGEP) such as but not limited to, API 510 Pressure Vessel Inspection Code: In-service Inspection, Rating, Repair, and Alteration, (2014) section 9.3.4 for external inspections, when they failed to perform an adequate inspection on Pressure Vessel tank #2 containing 88,992 lbs. of Propane. (c) On October 16, 2024, and at times prior, at the RapidXchange facility in Brookshire, Texas, the employer did not follow Recognized and Generally Acceptable Good Engineering Practices (RAGAGEP) such as but not limited to, API 570 Piping Inspection Code: In-service Inspection, Rating, Repair, and Alteration of Piping Systems (2016) section 6.4 for Visual External Inspections, when it failed to perform adequate inspection on the process piping connected to pressure vessels containing propane.
