29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) 812 Main St, Toms River, NJ - The employer allowed employees to work from a fabricated frame scaffold approximately, 31 feet long, 26 feet high and 5 feet wide and working approximately 13 feet and 18 feet above the ground without the working surfaces fully planked or decked between the front uprights and the guardrail supports, thus exposing the employees to fall hazards. This condition was observed on or about October 17, 2024. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer than 3 feet to the insulated energized power lines: a) 812 Main St, Toms River, NJ - The employer allowed employees to work from a fabricated frame scaffold approximately, 31 feet long, 26 feet high and 5 feet wide which was erected within the minimum distance of 3 feet to the insulated energized (110/212v) service lines, thus exposing employees to electrocution hazards. This condition was observed on or about October 17, 2024. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

This condition was observed on or about October 16, 2024.
