29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles. At a residential worksite located at 2914 Sherman Ave, St. Joseph MO -- employees engaged in roofing duties by tearing off the old roof using tools such as hammers, pitch forks and other small hand tools were not wearing eye protection. This exposed employees to getting shrapnel and other debris in their eyes causing injury. This was documented on or about 10/16/2024.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a residential worksite located at 2914 Sherman Ave, St Joseph MO -- employees engaged in roofing duties on a 14:12 pitch roof were either not using any personal fall arrest system, were using a body belt that is not allowed, and/or were using a safety harness with body belt system so that they were tied off in the front rather than the back. This exposed employees to fall hazards of 21.6 feet to ground level. This was documented on or about 10/16/2024.
