Cleveland Stone Companies LLC on or about October 17, 2024, at the location of 850 West River Road, Vermilion, Ohio 44089: a). The employer did not ensure employees tasked with servicing and maintenance tasks of Wilson gantry saw followed established energy control procedure to isolate and secure all potential hazardous energy exposing them to crushing hazards. b). The employer did not ensure employees tasked with servicing and maintenance tasks of and GMM Litox Saws 1-3 followed established energy control procedure to isolate and secure all potential hazardous energy exposing them to crushing hazards.

Due to this condition, employee tasked with operating GMM Litox Saws 1-3 from within the operating cell in proximity of the blade were exposed to lacerations and crushing hazards.

29 CFR 1910.184(c)(1):Sling(s) that were damaged or defective were used. a) Cleveland Stone Companies LLC on or about October 17, 2024 at the location of 850 West River Road, Vermilion, Ohio 44089: The employer failed to ensure damaged or defective nylon slings were not used to handle material. Due to this condition, employee using damaged nylon slings (Mazzella EE2 - 802 X 14') to hoist and transport material to the Wilson Saw staging area were exposed to crushing hazards.
