29 CFR  1926.501(b)(13):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.  On or about October 16, 2024, employees engaged in residential roofing activities, located at 11701 S 119th Ave, Papillion, NE 68046, were exposed to fall hazards in that the employees were working in excess of 6-feet while conducting residential roofing operations without fall protection.

29 CFR  1926.1053(b)(16):Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately.  On or about October 16, 2024, employees engaged in residential roofing activities, located at 11701 S 119th Ave, Papillion, NE 68046, were exposed to fall hazards in that the employees were accessing and egressing a roof using a ladder with visible structural damage.
