29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about October 16, 2024, at the above addressed jobsite, employees performing cleaning activities on the ground, working below employees conducting roofing operations were not wearing hard hats to protect against falling materials.

On or about October 16, 2024, at the above addressed jobsite, an employee performing residential roofing operations, thirteen (13) feet eight (8) inches, above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about October 16, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet two (2) inches, above the ground, were not protected from falls, thereby exposing employees to fall hazards.
