29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about October 11, 2024, on the roof of a residential building, employees were performing roofing work activities from the roof and were not in use of fall protection. a. On the south side of the roof, an employee was working at a height greater than six(6) to the ground below. b. On the north side of the roof, employees were working at heights that was more than six(6) feet to the ground below.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. On or about October 14, 2024, on the west side of the residential building above the garage, employees performing roofing work activities were not provided a safe means to access and egress the roof.
