The extension ladder that was used, did not extend at least 3 feet above the landing, on or about, October 18th, 2024.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). A) 12 Boxwood Lane, Rye - NY: An employee was exposed to fall hazards of approximately 13-feet while engaged in roofing work on a residential structure. The employee was not protected by any type of fall protection system, on or about, October 18th, 2024. REPEAT INFO: Jaro Danc, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1655385, citation number 1, item number 1 and was affirmed as a final order on September 30th, 2024, with respect to a workplace located at 12 Boxwood Lane, Rye - NY.
