29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which the employee was or would be involved: a) Worksite, heifer barn, pulleys and drive belt associated with the Westfield grain auger: On or about October 13, 2024, employees did not receive instruction on the safe operation of the Westfield grain auger and were exposed to nip point hazards due to an unguarded pully and belt.

29 CFR 1928.57(a)(7): The employer did not protect employees from coming into contact with hazards created by moving machinery: a) Worksite, heifer barn, pulleys and drive belt associated with the Westfield grain auger: On or about October 13, 2024, an employee was using a Westfield grain auger that was powered by a motor and the belt and pulleys used to drive the grain auger were not guarded. The employee was exposed to a pinch point.
