29 CFR  1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  a) Lathe operator :   On or about October 24, 2024, and at times prior to, the employer exposed employees to noise dose at levels of 78.4% (equivalent to an 8-hour time weighted average TWA of 88.2 dBA) which is 1.56 times the Action Level of 50% (equivalent to an 8-hour TWA of 85 dBA) while operating the Maschinenfabrik Zuckerman Weinn Lathe and unknown manufacturer for the Belt Sander. Results were calculated for a 237-minute sampling period.  The employer failed to provide an annual audiometric test to the employee.  b) Pop up saw operator :   On or about October 24, 2024, and at times prior to, the employer exposed employees to noise dose at levels of 85.2% (equivalent to an 8-hour time weighted average TWA of 88.8 dBA) which is 1.7 times the Action Level of 50% (equivalent to an 8-hour TWA of 85 dBA) while operating an unknown manufacturer Pop-Up Saw. Results were calculated for a 440-minute sampling period.  The employer failed to provide an annual  audiometric test to the employee.

Results were calculated for a 447-minute sampling period.  The employer failed to provide and post a copy of the standard to the employee.  c) Pop up saw operator :   On or about October 24, 2024, and at times prior to, the employer exposed employees to noise dose at levels of 85.2% (equivalent to an 8-hour time weighted average TWA of 88.8 dBA) which is 1.7 times the Action Level of 50% (equivalent to an 8-hour TWA of 85 dBA) while operating an unknown manufacturer Pop-Up Saw.
