29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a. On or about October 21, 2024, at the above addressed jobsite, an employee working on the ground, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.   The Byron Heredi dba Byron Heredi was previously cited for a violation of this occupational safety and health standard 1926.100(a), which was contained in OSHA inspection number 1522472, citation number 1, item number 1 and was affirmed as a final order on June 14, 2021, with respect to a workplace located at 118 Albion Ave. Park Ridge, IL 60068.

On or about October 21, 2024, at the jobsite located at 9908 W 145th Pl, Orland Park, IL 60462, an employee was exposed to eye injury while operating a pneumatic nail gun without wearing eye protection while installing roof shingles.   The Byron Heredi dba Byron Heredi was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1522472, citation number 1, item number 2 and was affirmed as a final order on June 14, 2021, with respect to a workplace located at 118 Albion Ave. Park Ridge, IL 60068.

Byron Heredi dba Byron Heredi was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1522472, citation number 2, item number 1 and was affirmed as a final order on June 14, 2021, with respect to a workplace located at 118 Albion Ave. Park Ridge, IL 60068.  Byron Heredi dba Byron Heredi was previously cited for a violation of this occupational safety and health equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1505386, citation number 1, item number 1 and was affirmed as a final order on February 17, 2021, with respect to a workplace located at 858 Center Court, Units A, B, C and D, Shorewood, Illinois 60404.  Byron Heredi dba Byron Heredi was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1497852, citation number 1, item number 1 and was affirmed as a final order on February 17, 2021, with respect to a workplace located at 21227 S.
