29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions in 29 CFR. a) Sealer Line #25: On or about 10/14/204, when diagnostic testing was undertaken by a maintenance employee, appropriate guarding and barriers were not in place to allow him to see the problematic area and diagnose the problem while the machine was operating and before any work was supposed to be undertaken.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Sealer Line #24, 25 and 26: On or about 10/14/2024 and at times prior too, employees were exposed to struck-by and caught-in hazards due to rotating shafts and other moving components, in that the employer did not adequately guard the entire line as the guard fencing used down the line was not secured so that it could not be opened without any type of tool.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): (a) Sealers Line #24, 25 and 26: On or about 10/14/2024 and at times prior too, warning signs were not placed on the guards of chains and sprockets or fencing located throughout each line to inform employees to keep hands clear when running, and to lock out the machine before reaching into hazardous machinery.
