Employees engaged in material handing activities in the warehouse located at 18905 Killcreek Road, Edgerton, Kansas using the Toyota forklift, were exposed to struck-by hazards in that comprehensive forklift training was not provided to all employees prior to permitting forklift operation.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision: (a) Employees were exposed to fire and electrical hazards. Employees engaged in using the General battery charger to charge powered industrial trucks at charging station 26-154 in aisle 36 located at 18909 Killcreek Road, Edgerton, Kansas were exposed to burn hazards due to the charger door being damaged, thus exposing employees to energized components. (b) Employees were exposed to fire and electrical hazards. Employees engaged in using the EnerSys battery chargers (R3414C and R2857C) to charge powered industrial trucks in aisle 36 located at 18909 Killcreek Road, Edgerton, Kansas were exposed to burn hazards due to an opening on the side of the cabinet not closed, thus exposing employees to energized components.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place or were not maintained in a legible condition: Employees were exposed to struck-by hazards. Employees engaged in material handing activities using the Toyota forklift R3515, R3868 (#10), and #9 were exposed to stuck-by hazards in that the following markings were not in place and legible: a) Control levers for operating the mast was not legibly marked by not highlighting the indentations embedded in the rubber knobs. b) The control lever that moves the mast from side to side was not marked.
