29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a)Gray Construction Company at 151 North Maple, Watertown, SD 57201: On or about and at times prior to October 21, 2024, the exposing employer did not ensure frequent and regular site inspections were conducted to ensure that employees were protected from leading edge falls while installing sheeting on a multi story commercial business. This condition exposed the employee to fall hazards greater than six feet.

29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record to include the name or other identity of the employees trained in fall protection, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer which exposed employees: (a)Gray Construction Company at 151 North Maple, Watertown, SD 57201: On or about and at times prior to October 21, 2024, the exposing employer did not ensure that employees were properly trained and documented on the hazards of falls or how to identify hazards prior to being exposed while conducting roof sheeting operations on a multi story commercial property. This condition exposed the employee to fall hazards greater than six feet.
