29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) New residential construction on Lot 167 at 5140 Kongoni Court, Cumming, GA 30041, Upper roof: On or about 10/22/2024, the employer exposed employees to falls of 19 feet, 5 inches to the lower ground level, in that a fall protection system was not provided to employees engaged in roofing activities on a 6/12 pitched roof. b) New residential construction on Lot 167 at 5140 Kongoni Court, Cumming, GA 30041, Lower roof: On or about 10/23/2024, the employer exposed employees to falls of 10 feet to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 8/12 pitched roof.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) New residential construction on Lot 167 at 5140 Kongoni Court, Cumming, GA 30041, Lower roof: On or about 10/23/2024, the employer exposed employees to fall hazards of 10 feet to the lower ground level, in that the extension ladder used for access was not extended or secured to the structure to allow safe access on and off the roof while employees were engaged in roofing activities on a 8/12 pitched roof.
