29 CFR  1910.178(m)(6):A safe distance was not maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car. Trucks cannot  be used for opening or closing freight doors.  a) Receiving loading dock: An employee was operating a Yale forklift model GLC030VXNURV082 serial number C809V03809E near the edge of the loading dock.  The back left tire went over the edge of the dock as the employee was turning the forklift causing it to fall to the ground below crushing the employee, on or about 10/23/24.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
