29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping or has guardrails which block employee access to the cantilevered end:   On or about, October 21, 2024, on a scaffold, employees were performing masonry construction activities such as but not limited to moving masonry materials to an upper level, where the end of a scaffold plank extended more than 12-inches over its support.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  On or about October 22, 2024, employees were performing masonry construction activities such as but not limited to framing blocks around a door opening, without a ladder or other safe means of access, in the following instances:  a)	On the first-floor scaffold. b)	On the second-floor scaffold.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  On or about October 21, 2024, on the north side a scaffold, employees were performing masonry construction activities such as but not limited to framing blocks around a door opening where braces were not installed on the scaffold.

29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    On or about October 21, 2024, on a scaffold, employees were performing masonry construction activities such as but not limited to framing blocks around an unguarded wall opening at a height greater than 6-feet to the lower level.
