29 CFR 1915.116(b): Loads were not safely rigged before being hoisted: a) USNS Lt John P. Bobo: On October 23, 2024, the employer attached rigging gear to pre-installed U-bolts that failed to support the elevator, thus causing the elevator to fall, striking a subcontractor?s employee. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Bobo: On 23 October 2024, employees were to install 6x6 wood shoring underneath the ship's elevator car while it was suspended by rigging. b) USNS Lt John P. Bobo: On 23 October 2024, employees were exposed to a struck by hazard while working underneath the ship's elevator car, removing wire rope, while it was suspended by rigging. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
