29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees. On or about October 23, 2024, the employer failed to require an employee wear the proper personal protective equipment while terminating wires inside an energized 480 volt switchgear, exposing the employee to an arc flash. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. On or about October 23, 2024, the employer did not protected an employee against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means, exposing the employee to 480 volts. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
