On or about October 23, 2024, the employer failed to require an employee wear the proper personal protective equipment while terminating wires inside an energized 480 volt switchgear, exposing the employee to an arc flash.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
