29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. Location: Job site; On or about 10/03/2024, a worker suffered a workplace accident where they reportedly suffered a finger amputation on a saw. This incident was reported to OSHA by the Massachusetts Department of Public Health on 10/24/2024. The employer failed to report this incident to OSHA within 24-hours of its occurrence.
