29 CFR  1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a)  At the facility, on or about October 23, 2024 - A continuing, effective hearing conservation program was not instituted when employee noise exposures equaled or exceeded an 8-hour time weighted average sound level (TWA) of 85 dBA.  An employee, designated as a Laminator Operator, had an exposure to continuous noise at a dose of 97.5% during a 401 minute sampling period on January 17, 2025.  The exposure is approximately 1.95 times the OSHA action level of 85 dBA.  This is equivalent sound level of 89.8 dBA .  b)  At the facility, on or about October 23, 2024 - A continuing, effective hearing conservation program was not instituted when employee noise exposures equaled or exceeded an 8-hour time weighted average sound level (TWA) of 85 dBA.  An employee, designated as a Laminator Operator, had an exposure to continuous noise at a dose of 73% during a 434 minute sampling period on January 17, 2025.   The exposure is approximately 1.46 times the OSHA action level of 85 dBA.  This is equivalent sound level of 87.7 dBA .  c)   At the facility, on or about October 23, 2024 - A continuing, effective hearing conservation program was not instituted when employee noise exposures equaled or exceeded an 8-hour time weighted average sound level (TWA) of 85 dBA.  An employee, designated as a Laminator Operator, had an exposure to continuous noise at a dose of 88.5% during a 395 minute sampling period on January 17, 2025.   The exposure is approximately 1.77 times the OSHA action level of 85 dBA.  This is equivalent sound level of 89.1 dBA .

An employee, designated as a Laminator Operator, had an exposure to continuous noise at a dose of 88.5% during a 395 minute sampling period on January 17, 2025.   The exposure is approximately 1.77 times the OSHA action level of 85 dBA.  This is equivalent sound level of 89.1 dBA .

An employee, designated as a Laminator Operator, had an exposure to continuous noise at a dose of 88.5% during a 395 minute sampling period on January 17, 2025.   The exposure is approximately 1.77 times the OSHA action level of 85 dBA.  This is equivalent sound level of 89.1 dBA.
