29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located at 5116 North Marblefalls St. Wichita, KS 67219, employees engaged in residential re-roofing activities were exposed to fall hazards of approximately 15 feet 9 inches from the eave of the roof in the back side of the house to the ground below in that fall protection training was not provided. The employer allowed his employees to perform roofing activities without first ensuring his employees were trained on the hazards associated with falls to include but not limited to setting up and inspecting fall protection exposing his employees to a fall hazard of up to 15 feet 9 inches to the ground below.

The ladder used to access the roof was observed to not extended at least 3 feet above the eave and was not secured to the structure.
