29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) New residential construction located on Lot 83 Dahlgreen Ct, Cumming, GA 30041, Roof: On or about 10/25/2024, the employer exposed employees to fall hazards of approximately 22 feet, 2-inches from the 12/12 pitched roof in that the employer allowed employees to use rock climber type waist harnesses in place of full body harness with no connection to D-Ring.

29 CFR 1926.552(a)(1):The employer shall comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. a) New residential construction located at Lot 83 Dahlgreen Ct, Cumming, GA 30041, Front side of structure: On or about 10-25-2024, and at times prior, the employer exposed employees to struck-by hazards, in that the employer did not follow the manufacturer's specifications of the Transporter Platform Hoist as the wire cable it was manufactured with was replaced using a rope.
