29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder: (a) On or about 10/23/2024, at 1467 Fraser Way, Farmington NY, 14425: Employees were using an extension ladder to get off the roof and the ladder was not extended 3 feet above the edge of the roof exposing employees to a fall hazard of 16 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about 10/23/2024, at 1467 Fraser Way, Farmington NY 14425 : Employees were installing roofing material on a house with multiple pitched roofs estimated to be 6:12, 8:12, and 10:12 without fall protection exposing employees to fall hazards ranging from an estimated 9 feet to 25 feet above the ground. Cameron Roofing was previously cited for an equivalent violation of the safety and health standard 1926.501(b)(11), which was contained in OSHA inspection 1613141, citation number 1, item number 1 and was affirmed as final order on 9/29/2022, with respect to a workplace located at 40 Canfield Place, Rochester NY, 14526.
