29 CFR  1910.178(m)(5)(iii): The employer did not ensure that powered industrial truck operators fully lowered load engaging means when the operator dismounted and  was within 25 feet of the truck still in his view.  a) On or about October 25, 2024, and at times prior, the employer did not ensure that when the operator dismounted and was still within view of the Clark GTS25 LP powered industrial truck, that the load engaging means was fully lowered while loaded when loaded with a bundle of Weyerhaeuser lumber weighing 3,542 pounds, inside a railroad car.

29 CFR  1910.178(q)(7): Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:  a) On or about October 25, 2024, and at times prior, the John Deere Loader 1LY244JXLZB036356 was in service, and the panel covering the electrical components was missing which exposed employees to electrical hazards and adversely affected the safety of the vehicle.  b) On or about October 25, 2024, and at times prior, the Daewoo D70S powered industrial truck was in service, and the tires were missing large chunks, the rubber was separating and cracking around the rim, the front windshield had a large crack impacting the visibility of the driver, the seat was ripped, and the seatbelt did not retract which adversely affect the safety of the vehicle.
