29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Rockville United Methodist Church roofing project, 142 Grove Street, Vernon, CT: On or about October 28, 2024, the employer did not ensure that head protective equipment was used when employees were exposed to impact hazards of falling material and/or tools when working in the fall zone of the roof eve from an approximate height of 11 feet to the ground.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  Rockville United Methodist Church roofing project, 142 Grove Street, Vernon, CT: On or about October 28, 2024, employees were exposed to a fall hazard of approximately 11 feet from the eave of the roof to the ground while performing roofing work activities without fall protection on an 8/12 pitch roof.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were rigged such that an employee could free fall more than 6 feet (1.8m) or contact a lower level:  Rockville United Methodist Church roofing project, 142 Grove Street, Vernon, CT: On or about October 28, 2024, employees were exposed to fall hazards while  performing roofing work on a 10/12 pitch roof while the vertical lifelines were not rigged in a manner that would prevent the employees falling from an approximate height of 11 feet from the eave of the roof to the ground.
