29 CFR 1910.333(b)(2)(iv)(B): A qualified person shall use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized. The test shall also determine if any energized condition exists as a result of inadvertently induced voltage or unrelated voltage back feed even though specific parts of the circuit have been deenergized and presumed to be safe. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately after this test:   A) Norfolk Naval Base, 1968 Gilbert St. Building W-143 Norfolk VA 23511 on 10/19/ 2024: Employees worked replacing drive rods in Air Switch Vault 1A and 2A.  Energy control testing procedures were performed, but it failed to properly verify that circuit elements and equipment parts were deenergized.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to electrical shock hazards while working on an electrical switch, where the circuities, that deenergize the equipment, were not staying in the open position allowing for the potential of reenergization.  a)  Norfolk Naval Base, 1968 Gilbert St. Building W-143 Norfolk VA 23511: On or about 10/19/2024, the agency failed to perform  annual visual inspections and merger readings of the metal-enclosed switchgear as specified by the manufacturer, resulting in a drive rod failing to stay in the open position, thus causing third-degree burns to an employee.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
