29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). On or about October 25, 2024, employees of IWR North America, LLC were installing insulated metal paneling on a horizontal beam in the 4th-floor plenum area. An employee was using the extension ladder at an angle such that the horizontal distance from the top support to the foot of the ladder was less than one-quarter of the working length of the ladder.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. On or about October 25, 2024, an employee of IWR North America working at One Barnes Jewish Hospital Plaza in Saint Louis, Missouri was exposed to fall hazards. The ladder he was working from was not secured to prevent accidental displacement while placed in front of a doorway.
