29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: The employer did not ensure employees were protected from head injury due to impact or flying objects while working beneath overhead loads in a trench. The condition was observed on or about 28 October 2024, when an employee was observed working in a trench at 3057 Lions Den Road in Imperial, MO. A Komatsu PC128us excavator was removing earth material from the bottom of the trench and the loaded bucket was within close proximity and above the exposed employee.

The employer did not ensure employees were protected from bodily injury due to impact or flying objects while working inside of a trench without a protective safety barrier. The condition was observed on or about 28 October 2024, when an employee was observed working in a trench at 3057 Lions Den Road in Imperial, MO. The employee was working in the trench during excavation operations and in close proximity to the spoil pile and excavator.

The condition was observed on or about 28 October 2024, when an employee was observed working in a trench at 3057 Lions Den Road in Imperial, MO. The trench measured 6 feet in depth where an employee was exposed to engulfment hazards while preparing to install 12" storm pipe.
