29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about October 28, 2024, at the above addressed jobsite, an employee performing roofing operations on the roof below an employee conducting roofing operations on the upper level was not wearing a hard hat to protect against falling materials.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about October 28, 2024, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a. On or about October 28, 2024, at the above addressed jobsite, employees conducting roofing operations, approximately nine (9) feet two (2) inches above the ground, were not protected from falling, thereby exposing the employees to fall hazards.
