29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) 401 Park Rd. Hardin, MT: On or about October 28, 2024, and at times prior, employees were exposed to fall hazards of approximately 11 feet while being lifted and transported in a man basket that was not secured to the forks of the onsite rough terrain forklift.

Hardin, MT: On or about October 28, 2024, and at times prior, employees were exposed to fall hazards of approximately 11 feet while utilizing a home made man basket to access upper walking working surfaces to perform roofing and framing operations.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) 401 Park Rd, Hardin, MT 59034: On or about October 28, 2024, and at times prior, employees were exposed to fall hazards of approximately 18 feet to the concrete ground below while replacing the roofing membrane on a commercial roof.
