29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 21, 2024, on the west side of the project job site, employees worked near a Schwing 36-meter concrete pump truck parked within 30 feet from overhead electrical power lines rated at 14.4KV's and within the slewing radius of the boom, without the required training to recognize the hazards in their industry.

29 CFR 1926.600(a)(6)(i): For lines rated 50 kV or below, minimum clearance between the lines and any part of the crane or load shall be 10 feet: On or about October 21, 2024, on the west side of the project, utilizing a concrete pump truck with the boom extended for a concrete pour, the boom made contact with an overhead transmission line rated at 14.4KV's electrifying the concrete pump truck and concrete supply vehicle.
