29 CFR 1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. In the main production room, located at 8370 Philips Highway, Jacksonville, FL: On or about October 26, 2024, the employer exposed employees to amputation hazards, in that the carton sealer (202-3) used to seal card board boxes had improperly installed/designed card board installed as a guards.
