29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On or before 10/29/2024, the employer did not ensure that the fork extensions used in the lawn and garden areas were approved by the manufacturer. The change in load center was not reflected in the data plates. This condition exposed employees to overloaded forklifts.
