29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about October 29, 2024, employees performing tear off work on a 8:12 pitched roof, on the north side of a residential home, were not connected into a fall protection system. This exposed employees to a fall hazard of approximately 25 feet from roof to the ground below. (b) On or about October 29, 2024, an employee performing tear off work on a 8:12 pitched roof, on the south side of a residential, was not connected into a fall protection system. This exposed an employee to a fall hazard of approximately 18 feet from roof to the ground below. The Remodeling Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1712111, citation number 1, item number 1, and was affirmed as a final order on April 24, 2024, with respect to a workplace located at 112 Industrial Road in Beaver Dam, WI 54911 .

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about October 29, 2024, employees performing tear off work and installing shingles on an existing residential home, used a Gorilla Aluminum adjustable ladder to access the roof that was not erected three (3) feet above the upper landing. The Remodeling Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1712111, citation number 1, item number 2, and was affirmed as a final order on April 24, 2024, with respect to a workplace located at 112 Industrial Road in Beaver Dam, WI 54911 .
