29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about October 29, 2024, employees were exposed to struck by hazards while installing shingles with pneumatic nail guns on a commercial roof located at 5548 State Road PARMA, OH 44134, without wearing any form of eye or face protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about 10/29/2024, the employer and GC did not ensure employees engaged in the installation of shingles from a 06/12 pitched commercial roof located at 5548 State Road PARMA, OH 44134, 13.6 feet above the grass covered ground were protected by guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 29, 2024, employees were exposed to a fall hazard of 13.6 feet while completing roofing work on a commercial building located at 5548 State Road PARMA, OH 44134. Employees were using an extension ladder to access the roof, and the employer did not ensure it extended at least 3 feet above the upper landing surface.
