29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 29, 2024, the subcontracted employees were exposed to a fall hazard of 13.6 feet while completing roofing work on a commercial building located at 5548 State Road PARMA, OH 44134. Subcontracted employees were using an extension ladder to access the roof, and the controlling employer did not ensure it extended at least 03 feet above the upper landing surface.
