Front of the building: Multiple extension cords were connected-in-series (daisy chained) providing power to a grinder. Employees were exposed to fire and electrical hazards.  b. Second Floor: An outlet box was attached to the end of an extension cord to power additional extension cords. Employees were exposed to fire and electrical hazards.
