29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Stryker Farmer's Exchange Company located at 114 South West Street in Stryker, Ohio; On or about July 29, 2024 the employer did not ensure that each powered industrial truck operator successfully completed the training and evaluation, specified by OSHA PIV training standards prior to permitting employee(s) to operate the Toyota Forklift Truck (model 52-6FGU30 serial no. 60576). Failure to provide the required training and evaluation to PIV operators will expose the operator, as well as others working in and around forklift operating areas to struck-by injuries, tipping, and rollover incidents.

29 CFR 1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Stryker Farmer's Exchange Company located at 114 South West Street in Stryker, Ohio; On or about October 14, 2024 the employer did not provide employees who wear filtering facepiece respirators voluntarily with the information contained in Appendix D of 29 CFR 1910.134. Employees voluntarily using the 3M 8210 dust masks/respirators were not adequately informed of the potential hazards and limitations of use.
