An employee was standing on a ladder when he fell off the ladder, he hit his head on the ground and suffered a fatal injury.

29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. a) Jobsite, 300 Shadeland Avenue, East Pittsburgh, PA 15112: On or about October 17th, 2024, the employer failed to ensure ladders were inspected before initial use. The manufacture's stickers on the ladder were marred and defaced resulting in illegible inspection instructions.

Jobsite, 300 Shadeland Avenue, East Pittsburgh, PA 15112: On or about October 17, 2024, an employee fell off a ladder, hit his head and was hospitalized. Five days later the employee passed away. The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization.
