Facility Wide- On or about October 28, 2024, the employer did not ensure that employee(s) who operate powered industrial vehicles, including the sit-down Liquified Petroleum Toyota forklift(s), Model: 8FGCU25 Serial No. 63703, Serial No. 68116, and Serial No. 13984, were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
