29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) On or about October 30, 2024, employees which lock-out machinery/equipment, were not provided with a hazardous energy control program that included training, procedures, and periodic inspections.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about October 30, 2024, the Hwacheon 180B CNC lathe's interlock was not operable, in that the key for the interlock was removed and placed directly into the receiver, allowing the machine to operate with the door in the open position. This exposed employees to amputation and entanglement hazards.   (b) On or about October 30, 2024, the DMC DL 6th CNC lathe's interlock was not operable, in that the key for the interlock was removed and placed directly into the receiver, allowing the machine to operate with the door in the open position. This exposed employees to amputation and entanglement hazards.   (c) On or about October 30, 2024, the Sharp 1340VS lathe's chuck was not guarded to protect the operator from flying debris and entanglement hazards created by the chuck.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) At this location, employees engaged in metal parts manufacturing and tool and die work, were exposed to hazardous materials, such as but not limited to, oils and lubricants. The employer had not developed and implemented a program which would address labeling and other forms of warnings on hazardous material containers, Safety Data Sheets, and employee information and training on the hazards associated with the hazardous materials utilized on site, and is consistent with Globally Harmonized System (29 CFR 191200(a)(1)).  The written program must also contain the following: 1. A list of all hazardous materials on site; 2. The methods the employer will use to inform employees of the hazards associated with non-routine tasks involving hazardous materials, such as spills; 3. The hazards of the materials contained in piping that is not labeled, and; 4. The method the employer will use to inform other employers (contractors) of the chemicals their employees might be exposed to while performing duties at the site.
