On or about October 31, 2024, during a construction project on the roof of a residential building located at East 89th Street, CLEVELAND, OH 44106, the employer did not ensure that employee(s) were protected from possible danger of eye injuries from impact and/or flying debris by operating pneumatic nail guns without the use of eye protection.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about October 31, 2024, , during a construction project on the roof of an residential building located at East 89th Street, CLEVELAND, OH 44106, the employer did not ensure employees working on a low slope roof were protected from a fall of greater than 24 feet 3 inches by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.
