29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) a) 14359 Fillmore St., Thornton, CO: On or about October 29, 2024, and at times prior, as the controlling employer, Summit Solar & Roofing Inc. did not provide evidence that an effective inspection of the jobsite was conducted. The employer did not adhere to their safety program that requires a safety inspection to be completed on every jobsite prior to work. This condition exposed employees to a fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 14359 Fillmore St., Thornton, CO: On or about October 29, 2024, and at times prior, as the controlling employer, Summit Solar & Roofing Inc. did not ensure their subcontractor, Hugo Enriquez Gonsales protected their employees from injury in that employees installing new shingles on a one-story residential home did not utilize a fall protection system. This condition exposed employees to a fall hazard of approximately 15 feet to the ground below.
