On or about October 30, 2024, near 125 West Needles Street in Bixby, Oklahoma employees utilized the crossbars on the scaffolding as a means of access and egress for the scaffold when no other access or egress method was provided.

On or about October 30, 2024, near125 West Needles Street in Bixby, Oklahoma employees were not protected from falls when working near a window opening under construction.
