29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail:  a) On October 28, 2024, the guardrail system used as a barrier to prevent falls from the edges and sides of an elevated hatch platform was not secured to the structure with fasteners. While working from the platform, an employee grabbed the railing to assist in standing, the guardrail failed, resulting in the employee falling approximately 14 feet from the platform to the lower level.
