29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) Pyramid ETC Companies, LLC dba ETC Companies LLC, 109 Sunny Lane, Torrington, CT 06790; On and before October 31, 2024, and at times prior, the employer did not ensure that employees of JR Complete Construction were safe while conducting roofing work, by conducting frequent and regular inspections of the job site by a competent person in order to prevent and detect safety and health hazards. Employees were permitted to work without proper means of fall protection and were exposed to fall hazards of approximately 17 feet.
